New Patient Information
If you are a new patient, “Welcome!”, we look forward to meeting all your dental needs! Use the link to download our new patient forms and be sure to fill out the forms completely. Please try to arrive 10 minutes early for your appointment.
A couple of things to note:
- If the patient is under age, the patient’s legal guardian must bring the child.
- Pre-medicate prior to your appointment if directed by your doctor.
At our Gilbert dental office ‘Our patients are our family’. We know that a trip to the dentist isn’t fun, but we do our best to make your experience with us as positive as possible. We have movies in the waiting area, as well as flat screen televisions in each operatory so you can sit back and relax during your dental treatment. We also have complimentary toothpaste and toothbrushes available in our refresh room.
